Answers at a Glance

What are Botox · Filler · Fat-Dissolving Injections, and and who is it right for?

Botox, fillers, and fat-dissolving injections are non-surgical treatments that refine facial contours without surgery. Masseter Botox for a square jaw reduces the volume of a thick jaw muscle (masseter), fillers add volume to hollow areas such as the chin, lips, and corners of the mouth, and fat-dissolving injections (contouring injections) reduce fat along the jawline and cheeks. For patients who come to The Barunface to discuss contouring surgery but whose concern is caused by muscle or fat rather than bone, we recommend this treatment first, and we also use it as a supportive treatment before and after surgery.

Recommended for You If

  • Those whose face looks square due to thick jaw muscles
  • For those whose chin is slightly recessed or who lack volume in the corners of the mouth or lips
  • For those whose facial contour looks dull due to fat along the jawline and cheeks
  • Those concerned about forehead, glabella, or eye-corner wrinkles
  • For those who want to try a non-surgical approach before contouring surgery

Square jaw Botox or square jaw surgery: which is right for me?

If the side of your jaw bulges when you clench your teeth, muscle is the cause, so Botox comes first. If the jaw still looks angular when relaxed, bone is the cause, and Botox has limited effect. At your consultation, we check by touch and tell the difference with a CT scan.

What is filler used for?

Hyaluronic acid filler is placed in the chin tip (1cc), lips (1cc), and mouth corners (1cc) to enhance volume and contour lines. Chin tip filler works for a mildly recessed chin, but large amounts can look unnatural, so we recommend surgery for bone-related problems. Rejuran (2cc) is intended for skin regeneration.

Do fat-dissolving injections (contour injections) work?

It may help reduce subcutaneous fat in the jawline and cheeks, and is usually performed in several sessions at 2–4 week intervals. If the cause is bone or muscle rather than fat, the effect is limited. In one consultation case, a patient wanted cheekbone reduction, but because fat was the cause, contouring injections and Shurink were recommended instead.

When Dr. Lee Recommends Treatment

Whether we recommend Botox, filler, or fat-dissolving injections is decided by diagnosis, not by photos or trends. Below are the criteria the director actually uses in consultations, in the director's own words.

The goals and direction the patient hopes to achieve through surgery must match the goals and direction of the surgeon performing it.

— Dr. Sang-Hoon Lee · Director's written response, October 2026

I try to be straightforward and plain: I explain why something can be done and why something cannot.

Consultations where we did not actually recommend the treatment

Even among patients who come with the same concern, once the skeletal condition and the desired change are considered, there are cases where treatment is not the answer. This is the conclusion of a consultation in which the doctor made that judgment.

Because the cheekbone prominence is not severe and the changes achievable through skeletal surgery are limited given the proportions of your face, we do not actively recommend cheekbone reduction surgery or orthognathic (double jaw) surgery.

Tests and Preparation Before Your Consultation

For non-surgical treatments, too, we first look at the cause. If the cause is bone, non-surgical treatment may not deliver the change you hope for, which is why we tell you so.
